The Collective Heartbeat
Four counties. Four models. One shared mission.
The Learning Collaborative is where the counties come together, not to report to each other, but to learn from another. Each Collaborative has built something distinct, shaped by its own community, partners, and data. This is the space where those experiences become shared knowledge. They discuss what’s working, where things are hard, and what one county figured out that another can use.
This year, the full group is convening twice, once virtually and once in person. Next year, as Richmond and Dougherty Counties come online, they join the table too. The Learning Collaborative grows as the initiative grows, and the knowledge built here stays with every county long after the grant ends.

Between these cross-collaborative convenings, the conversation keeps going. The GA Heart Health Forum is a private, online community for Collaborative members across all four counties to share updates, post upcoming events, exchange resources, and stay connected between meetings. Access the Forum
